For one, there actually seems to be a brains behind the operation and they changed the poison being given to him through his ventilation system to a stronger kind along with slipping drugs onto the scrapy bread he's given which shakes his immune system and takes a toll on his body. And two, it seems that a gang raid is due to the people keeping him captivate.
